Surrender a dog

Surrendering a dog to Indi Lab Rescue

If you can no longer provide for your dog we may be able to help.

Please complete form below. We MUST have this form and 3 CURRENT PHOTOS of your dog prior to calling you.


Copy the text below and paste it into the body of an email,  type your answers next to each question. Attach 3 current photos of your dog where we can SEE the dog. Not the back or tail or top of the dog. We need to see your dogs face, body so you may want to tie your dog and take photos of them sitting on the ground to be at their level. Make sure you've answered everything and attached your photos and then send the email to info@indilabrescue.org with "Surrender my Dog" in the subject line.

Does your dog have any wounds, growths to be removed? 

Has your dog had a dental cleaning?

Does your dog have infected ears?

Is your dog spayed/neutered? Can you supply a spay/neuter certificate?

Is this dog up to date on rabies? Can you supply a rabies certificate?

is this dog up to date on other vaccines? Can you provide us a record of those from your vet?

Will you give permission to your vet to release medical records to us for our use in taking care of your dog?

How is your dog when left alone inside the house?

Is your dog possessive over food or toys?

Has your dog ever bitten or tried to bite a human OR a dog? What happened? Did you file a bite report?

Has your dog ever killed or injured a chicken, cat, or other critter?

Any habits or quirks we should know?
